Child Safety Seat Agreement

As a child safety seat recipient, I certify I will:

1. Abide by the National Child Passenger Safety Board Code of Conduct for CPSTs.
2. Set my SafeKids profile to “public”.
3. Complete the Car Seat Eligibility Certification Form and verify eligibility for each child safety seat distributed.
4. Distribute one child safety seat per child.
5. Record car seat check information using the National Digital Check Form.
6. Submit reporting documentation after distributing child safety seats.
